Islington CCG Care Closer to Home Integrated Network (CHIN)

Description

This a Voluntary ex ante Transparency Notice in pursuant to Regulation 99 of The Public Contract Regulations 2015. CHIN working will be enabled by the formation and management of patient registers that are shared between the CHIN practices. A register manager will use risk stratification tools to identify those patients that would benefit from a proactive intervention and these will be notified to the CHIN case manager who will oversee the initiation of proactive support to patients, along with subsequent case oversight. They will contact patients to make the initial assessment and if required they will develop a collaborative care plan and signpost to relevant services. Additional information: This is a Voluntary ex ante Transparency notice in pursuant of Regulation 99 of the Public Contract Regulations 2015. A 10 day standstil period will be in force between 14th of July 2017 and 24th July 2017.
