Work description
Medway Council's Adult Partnership Commissioning (APC) service are working for, and on behalf of, Medway Council and NHS Kent and Medway ICB, who wish to commission an Intermediate Care and Reablement Service (ICRS) for residents located in Medway.
The Intermediate Care and Reablement Service (ICRS) enables discharge from acute hospitals, mainly Medway Foundation Trust (MFT), through Pathway 1 (home-based intermediate care with reablement) and Pathway 2 (bed-based intermediate care with reablement) in line with the national discharge model with adaptability to any local discharge model redesign.
In terms of demand, the service sits between the acute hospitals, referring into the service, and Adult Social Care, assessing the need for care. The aim of the service is to prevent, reduce or remove the need for long term care by enabling residents to live independently for longer.
