## Description

This is a notice for social and specific services in accordance with Directive 2014/24/EU Article 74\. Surrey County Council are seeking to commission provider\(s\) to design and deliver an accessible tier 2 lifestyle weight management service\(s\) for children / young people between 5 and 19 years of age who are overweight or obese and their families to help them develop healthier lifestyles and reach and maintain a healthier BMI\. The service will form an integrated part of the planned local healthy weight / weight management care pathway\. The provider\(s\) will be required to deliver a tier 2 weight management service for children/young people and their families in each of our five CCG areas across Surrey\. This will be a two year contract\(s\) with the option of a one year extension\.
