Cardiac Rehabilitation Phase IV

To procure Aquaterra, The Borough of Islington's current provider, to continue their provision of Phase IV Cardiac Rehabilitation Service for sufferers of heart-related conditions after they have finished their care with the hospital. The existing service has created a long-term partnership and has established a clear Patient pathway into Phase IV rehabilitation programme. The service is a high impact service, keeping cardiac patients out of the hospital.
