## Description

To procure Aquaterra, The Borough of Islington's current provider, to continue their provision of Phase IV Cardiac Rehabilitation Service for sufferers of heart-related conditions after they have finished their care with the hospital\. The existing service has created a long-term partnership and has established a clear Patient pathway into Phase IV rehabilitation programme\. The service is a high impact service, keeping cardiac patients out of the hospital\.
