PP17153 - Dementia Community Support Service

The City of Wolverhampton Council wish to establish a co-ordinated and comprehensive post diagnosis support service that comprises of two components but operates as a single entity with a single point of contact. The single point of contact needs to be available to the public from 9.30am to 4.30pm Monday to Friday as a minimum.
