Description
L&Q invited proposals to provide a H&W programme to our residents. This will form as another arm to our all-encompassing support provision within the Independent Lives Team. This would offer an outreach service to our residents where by a diagnostic assessment can be completed within the resident's home or locally. With a person-centred approach, we required an organisation to use its experience and skills to help encourage residents to take up opportunities either locally to them or via the provider themselves remotely.
